It is difficult to guess what may be going on without more information & especially a physical examination in a case like this, but if she is truly unaware that she is defaecating, then there may be problems with the nerves around her anus and rectum. Your vet will probably need to perform a rectal
examination, to ensure that she has not got an accumulation of faeces in her rectum - this would require an enema to clear. Nerve problems are sometimes difficult to localise, as they can originate in the spinal cord, or in the local area affected.
